6. Equity FirstLine Credit Application. This is a fill-able application that you can complete right on your computer. IMPORTANT: Once the application is completed please remember to review all of the information you provided for accuracy. The electronic application will pre-fill the totals for you. Review your Assets and Liability totals for accuracy. Print two copies of the application and Proof of Insurance, and retain one copy of each for your files along with the provided disclosures. Once you exit, your application will not save as a completed application.

0. After completing the Equity FirstLine Credit Application, Authorization to Release Information and Proof of Insurance form (CC-632), please return these completed forms to us along with copies of the following documents:
1. Copy of homeowner's insurance policy
2. Copies of last two current pay stubs from employer or last two years federal tax returns, if self-employed
3. Long Form Trust Documents, if applicable

Additional information may be required if necessary to reasonably complete our credit analysis and to secure our mortgage position.
